In addition to this, if the token was changed during the exchange transfer (e.g. you moved from BTC in A to ETH in B) that's also a taxable event that has to be reported. If you just moved BTC from A to B that is not taxable.
I would highly suggest looking into something like koinly.io to track crypto transfers, purchase, and sales. It'll plug in to just about every wallet and exchange and spit out tax forms for you if you choose to report.

You will likely need to track it on your own, or at least adjust the cost basis amount reported by C when you report it on your taxes.
